Longitudinal Care Plans

Enroll patients on custom-tailored "longitudinal care plans" that can be easily adapted from a library of standard care plan templates for congestive heart failure, coronary artery disease, diabetes, COPD, hypertension etc. Patients can be enrolled in care plans with the care plan wizard and associated flow sheets are automatically enabled to track the patient's status over time and along with patient-specific goals and parameters. System-generated alerts for missed and upcoming visits and abnormal clinical measures can be viewed in the My Alerts section and can be optionally emailed to the appropriate care team member or patient.

Patient Outreach

Generate focused patient contact lists and communication templates to reach patients via email, SMS, smartphone or by letter at any step of the process from scheduling them for the initial consult to following up on an incomplete service and adjusting their care plan as needed when care plan goals are off track.
